When you visit our office for the first time, please complete any remaining forms, including the Authorization and Consent for Treatment form, if you haven't already done so on your smartphone or computer. To ensure a smooth registration process and timely care, please arrive 15 minutes before your scheduled appointment. This allows us to finalize your registration before you meet your new provider.
